TERMS AND CONDITIONS
1. To qualify for the $20 (USD) rebate, you must purchase a new eligible 2GB Crucial SODIMM memory module kit (part number: CT2KIT12864AC667) from the Buy.com Web site: www.buy.com.
2. All claims must be postmarked by 6/20/07. Each submission must be accompanied by (1) this rebate form, (2) your original dated receipt with qualifying purchase circled, (3) the original UPC code from the product packaging that your memory modules came in. No rebate will be processed without a properly completed rebate coupon. Sales order confirmation e-mails will not be accepted as proof-of-purchase. Crucial is not responsible for lost, stolen, or misdirected submissions. Void where
prohibited by law.
3. Limit one rebate per eligible Crucial SODIMM memory module kit per household. Submissions from P.O. box addresses will not be honored. Rebate valid only for United States addresses. Fraudulent submissions or multiple requests could result in prosecution under applicable laws. Rebate coupons are non-transferable to other retailers for distribution, to other end-users or customers. Not valid with any other offer. Offer good while supplies last.
4. After the rebate fulfillment form has been submitted, only product exchanges are permitted.
5. Please keep copies of all material submitted. All submitted materials become the sole and exclusive property of Crucial. Please allow 8-10 weeks for receipt of rebate check. Rebate checks will be paid in U.S. dollars only. Checks not cashed within 90 days of the issuance are void and cannot be reissued.
